The entire Bayly Family Practice team is committed to high standards of patient care. 

If you are a new patient and require an appointment, kindly click New Patient Questionnaire and fill the questionnaire. Complete it with as much details as possible. This will enable us to have a good overview of your health and to update your records. It is not used to determine
whether you will be accepted as a patient or not.

Appointments can be booked through our website by clicking on the book appointment link. The link is also available on our Facebook page, and our Google page.

When coming in for your appointment, please aim to arrive at least 15 minutes early as there will/might be enrollment forms to complete and a practice policy to go through.

Our phone lines can get busy so bear with us if there are delays in answering your call. 

As always, we welcome your feedback and encourage you to visit our website regularly for updates. Routine inquiries can also be made through the website but please do not include any confidential information.

Frequently Asked Questions

What is expected of me as a patient?

Arrive 15 minutes early for all appointments. When you arrive, check in with the medical office assistant with your health card.

If you cannot make your scheduled appointment, call us to cancel the appointment.

When meeting with your doctor,  only discuss the concerns and issues that led you to book the appointment. Appointments are limited a single individual. Should you have inquiries for other family members, please book separate appointments.

What happens if I miss my appointments?

All missed appointments incur a $30 fee. Missing appointments puts our other patients at a disadvantage. This is because your spot could have gone to another person.

If you need to reschedule your appointment, please provide minimum 24 hours notice.

When should I see my doctor or go to the walk in clinic?

If a medical condition is life- or limb-threatening, you should visit the ER to receive hospital medical care.

Are you accepting new patients?

Yes we are accepting new patients! Please call the office to discuss your care needs.

Are doctor’s visits free?

Walk in visits and medical appointments with your family physician are covered under OHIP.  This means there is no cost to you.

However some services are not compensated by the provincial medical plan. To maintain the highest standard of care, our office policy is to charge for these uninsured services. Our fees reflect the recommended guidelines set by the Ontario Medical Association (OMA).

Pickering Walk-In Visits are available between Monday to Friday. You must call the clinic on the same day for walk-in availability.
